Hyperpigmentation & Melasma

Treating melanin disorders requires suppressing the melanocyte, not just removing the pigment. Heat is often the enemy.

The Crucial Distinction: Sun Damage vs. Melasma

Solar lentigines (sun spots) are localized areas of increased melanin due to UV damage. Melasma is a chronic, hormonally-driven condition characterized by symmetrical, blotchy hyperpigmentation (often on cheeks, forehead, and upper lip) that is highly reactive to heat and inflammation.

The Danger of IPL for Melasma

Intense Pulsed Light (IPL) generates significant heat in the skin. While it can successfully shatter surface pigment, the thermal energy often triggers a massive inflammatory response in melasma-prone skin. The pigment may look better for two weeks, only to rebound darker and deeper than before (rebound hyperpigmentation).

Hierarchical Treatment Protocol

Phase 1: Tyrosinase Inhibition (Topical)

Before any clinical procedure, the melanocytes must be chemically calmed to prevent Post-Inflammatory Hyperpigmentation (PIH).

  • Hydroquinone (4%): The gold standard. Competitively inhibits tyrosinase. Used in 3-4 month cycles.
  • Tranexamic Acid (Oral or Topical): Inhibits plasminogen activation, reducing arachidonic acid and prostaglandins (which stimulate melanocytes).
  • Cysteamine (5%): A powerful non-hydroquinone alternative.

Phase 2: Cold Mechanical & Chemical Exfoliation

Removing existing pigment without generating thermal injury.

  • Superficial Chemical Peels: Mandelic acid or low-percentage TCA.
  • Medical Microneedling: Used strictly for transdermal delivery of depigmenting agents (like tranexamic acid) into the dermis without heat.

Phase 3: Picosecond Lasers (The Exception)

If device intervention is necessary, picosecond lasers (like PicoSure or PicoWay) are used.

Unlike nanosecond or ablative lasers which rely on photothermal (heat) action, picosecond lasers fire in trillionths of a second. This relies on a photoacoustic effect—the sound wave literally shatters the pigment into dust without generating significant heat, making it much safer for melasma and darker skin types.
