Clinical Microneedling
Mechanical Collagen Induction Therapy (CIT) without thermal injury.
Mechanism of Action
Microneedling utilizes a motorized pen (e.g., SkinPen, Dermapen) oscillating solid surgical-steel needles to stamp thousands of micro-channels into the skin. This controlled physical trauma triggers the body's natural wound-healing cascade: Inflammation, Proliferation (fibroblasts produce new collagen and elastin), and Remodeling.
The Safety Profile Advantage
Because microneedling relies entirely on mechanical injury (cold steel) rather than photothermal injury (lasers) or electrothermal injury (RF), it does not heat melanocytes. This makes it the safest resurfacing modality for Fitzpatrick Skin Types IV-VI, carrying almost zero risk of Post-Inflammatory Hyperpigmentation (PIH) when performed correctly.
Clinical Depth Protocols
- 0.25mm - 0.5mm (Epidermal): Enhances transdermal delivery of topicals (like Vitamin C or Tranexamic Acid). Does not stimulate significant collagen.
- 1.0mm - 1.5mm (Papillary Dermis): The standard clinical depth for fine lines, overall texture, and mild collagen stimulation. Causes pinpoint bleeding.
- 2.0mm - 2.5mm (Reticular Dermis): Required for breaking down thick fibrotic tissue in deep acne scars (boxcar/rolling) or surgical scars. Highly uncomfortable; requires strong topical anesthetic.
PRP / Exosomes Integration
The micro-channels remain open for approximately 10-15 minutes post-procedure. Clinicians often use this window to apply Platelet-Rich Plasma (PRP) derived from the patient's own blood, or synthetic Exosomes. These growth factors penetrate the dermis, significantly accelerating healing and amplifying the collagen response.
