High-Intensity Focused Ultrasound (HIFU)

Targeting the surgical plane (SMAS) with acoustic energy.

Mechanism of Action

Unlike lasers (light) or RF (electrical current), HIFU uses acoustic sound waves. These waves pass harmlessly through the surface of the skin and converge at a specific, predetermined focal point beneath the surface. At this focal point, the intense friction generates heat (60-70°C), creating a precise Thermal Coagulation Point (TCP).

The SMAS Layer

The Superficial Musculoaponeurotic System (SMAS) is the fibrous network of tissue that plastic surgeons manipulate during a traditional facelift. HIFU transducers are calibrated to hit 4.5mm depth—specifically targeting the SMAS. The thermal injury causes the SMAS to contract instantly, providing a lift, followed by months of neocollagenesis.

Pain & Protocol Reality

HIFU (brand name Ultherapy) is notoriously painful. The energy is deposited directly over bones and near nerve bundles. Providers who claim it is a "lunchtime, painless" procedure are likely turning the energy down so low that it becomes sub-clinical (ineffective). Proper protocols require nerve blocks, Pro-Nox (laughing gas), or oral sedation.
